F-1 AT A REGULAR MEETING OF THE BOARD OF SUPERVISORS OF ROANOKE COUNTY, VIRGINIA HELD AT THE ROANOKE COUNTY ADMINISTRATION CENTER MEETING DATE: May 11, 2004 AGENDA ITEM: Request to appropriate $1,820,000 for the Community Policy and Management Team (CPMT) to provide funding for mandated services for youth with special needs SUBMITTED BY: John M. Hodge County Administrator COUNTY ADMINISTRATOR'S COMMENTS: SUMMARY OF INFORMATION: The Community Policy and Management Team (CPMT) is requesting an additional appropriation of $1,820,000 to cover the required expenses for FY 2003-04. Following the public hearing at today's meeting, staff respectfully requests the recognition of $1,000,000 new revenue from the State and an appropriation of $820,000 from the unappropriated fund balance of the general fund to cover the required expenses for this year. The new amounts have been included in the budget requests for the upcoming fiscal year. In 1992, the State implemented the Comprehensive Services Act (GSA) to become the funding source for youth with special needs and their families which had previously been funded by the Schools, Juvenile Court, and Social Services. The plan uses a collaborative effort to consider the identified needs of the child (Special Education for the Schools; Foster Care for Social Services; youth with charges before the Juvenile Court) to reduce the likelihood of duplicating or conflicting services and to seek cost effective means of educating or treating the child. There are many factors which must be considered in determining the status as a mandated service (whereby for Roanoke County, the State pays 56% and the locality pays 44%) or a non -mandated service where there is a shared match for the first $125,000 per year, and then any service is paid 100% by the locality. There are also some specialized and grant funds for certain qualified youth, but these also have limitations. As children with needs are identified, they are assigned a case manager who serves as the child's advocate and presents the needs to the Family Assessment and Planning Team (FAPT). This collaborative team is comprised of senior workers from the Departments of Health, Social Services, Schools, Court Services, a private provider of children services, and parent advocates. They listen to the identified needs and help to formulate a treatment plan based on their experience and knowledge of available programs. The treatments are aimed at providing the least restrictive conditions first with the aim of keeping the child in the home and community. Sometimes, the severity of need is greater than services in the home can manage and residential treatment programs are required. The goal is always to reunite the family and to provide the support services needed to allow the education and stabilization of the child. Frequently, this requires a combination of services which are decreased as the conditions improve. In cases where certain treatments are not effective, alternatives are considered. The CPMT is comprised of the directors of the core departments and other designated disciplines and they collaborate to approve funding and authorize treatment. The team also considers the utilization review to determine if the treatment goals are being accomplished. In many cases, the treatment extends over several years. In other cases such as foster care, the care may extend until the child is eighteen years old. Over the past few years as many of the service providers have attempted to become Medicaid providers, the overall cost of purchasing these services has increased dramatically, thus driving up the cost of the program. Children are being identified earlier and the presenting symptoms are more severe than in the past. Much of the increase for service is attributed to substance abuse, mental health issues, family dysfunction and domestic violence. As an example, over the last ten years the number of foster care cases handled by Social Services has grown from 35 per year to over 105 today. In instances where no other treatment monies are available to aid the child, the courts have placed the child under the umbrella of Social Services to qualify them for services. Some of the cases have minor costs for counseling, mentoring services, etc.; however, some of the severe placements in residential treatment facilities may cost nearly $200,000 per year far a child. In contrast, the family represented by one high dollar residential placement has moved out of Roanoke County which resulted in a savings for the current year. Attached is a listing of the cases for the current fiscal year and the projection of costs to be incurred by the CPMT. You will note that one child and family who had been placed in a residential treatment facility have moved from Roanoke County. The savings from this action have been calculated in this program. I FISCAL IMPACT: This fiscal year, the local share (match) for the program was budgeted at $1,153,000. However, based on current cases, it is anticipated that the deficit for the year will be $1,819,678. The share from the State will be $1,000,000 and $820,000 from local sources. STAFF RECOMMENDATION: As this request covers mandated cases, staff recommends that the Board authorize the appropriation of $1,820,000 in new money to the CPMT for FY 2003-04. Of this amount, $1,000,000 would be new State revenues and $820,000 would be taken from the unappropriated balance of the general fund for the local share. Mahoney County Attorney COUNTY ADMINISTRATOR'S COMMENTS: SUMMARY OF INFORMATION: This consent agenda item involves the acceptance of the following real estate for entrance improvements to the Vinton Business Center in the Vinton Magisterial District conveyed to the Board of Supervisors of Roanoke County, Virginia. a) Donation of 0.014 acres, together with a temporary construction easement of 0.023 acres, from Richard Limroth and Kelly B. Limroth (Deed Instrument # 200205163); Tax Map No. 71.07-1-21 as shown on a plat prepared by David A. Perfater, L.S. dated 12/6/2001, revised 12/19/01. STAFF RECOMMENDATION: Staff recommends acceptance of the donation of this real estate and that the County Administrator, or Assistant County Administrator, be authorized to execute such documents as may be necessary to accomplish this acceptance.
